We were attacked, Captain. Huge, furry creatures. I checked with astral anthropology, and they’re order 480-G, anthropoid, similar to life forms discovered on Hansen’s Planet, but much larger. 10... 12 feet in height.

Ensign O’Neal got a spear through the body before we even knew they were around. Lieutenant Immamura has a dislocated shoulder and severe lacerations, but he’ll make it. Captain, the creatures are all over the place. If Galileo is down on that planet, I—

Kirk

Thank you, Lieutenant. You better report to sick bay yourself.

Aye, aye, Captain.

Somehow Enterprise crewmember Kelowitz is always credited as Lieutenant Commander however this is not consistent with on screen evidence and not supported by dialogue either.

Lieutenant Kelowitz is introduced as a tactical officer in “Arena” where he wears a blue uniform and lieutenant stripes. When he beams down with the Captain, Mr Spock and two other officers he ends quickly in confirming the death of the other officer’s death to Captain Kirk. One very famous screenshot even does show he only wears one stripe as a lieutenant (see image).

The actor Grant Woods has another appearence as Kelowitz in “The Galileo Seven” this time his rank stripes are not seen because he is only visible via some sort of video conference. In that he is addressed several times as Lieutenant by Captain Kirk (see dialogue). Even though it is not uncommon to address a Lieutenant Commander by only calling him Lieutenant.

However he does appear again with the same costume in “This Side of Paradise”. Always with lieutenant stripes and never addressed as anything but lieutenant.

One explanation might be, that he was somehow confused with Lieutenant Commander Lang who died on Cestus III and was in return credited as Lieutenant in about every reference book.
